Initial Diabetes Assessment
A comprehensive first appointment to understand your health history, current management, medications, lifestyle, and goals. We'll identify what's working and where support is needed, and develop your personal education plan.
Diabetes Self-Management Education
Learn to understand your blood glucose levels, manage your medications, make informed food choices, incorporate physical activity, and recognise warning signs — all in plain, practical language.
Medication & Insulin Education
Guidance on how your diabetes medications work, how to take them safely, and what to watch for. Including insulin initiation support, injection technique, and device education (pens, pumps, CGM).
Ongoing Review & Monitoring
Regular follow-up consultations to review your HbA1c and blood glucose data, adjust your education plan, track progress toward your goals, and address any new challenges.
Pre-Diabetes & Prevention
If you've been told you have pre-diabetes or are at risk, early action makes a real difference. We'll help you understand your risk and implement lifestyle changes that can delay or prevent type 2 diabetes.
Telehealth Consultations
Can't make it in to Dayboro? All services are available via secure video or phone consult. Same high-quality, personalised care — from wherever you are in Australia.
GP & Team-Based Referrals
We work directly with your GP and multidisciplinary team. We accept Team Care Arrangements (TCA) and GP Management Plans (GPMP). Referral letters and reports provided.
Medicare Rebates Available
Sigrid is a Medicare-recognised Credentialed Diabetes Educator. Eligible patients may access rebates through a GP Management Plan (GPMP) or Team Care Arrangement (TCA). Ask your GP for a referral, or contact us for more information.
